The Hounslow GP Consortium CIC and its voluntary sector partners, Groundwork London and H4All CIO, are introducing a new management role to lead and develop the newly expanded Social Prescribing teams across Hounslow’s 5 Primary Care Networks (PCNs)

Social Prescribing Programme Manager

£32-36K p.a. plus contributory pension

The Social Prescribing Project Manager will work in collaboration with Hounslow Consortium (GP Federation) and the Clinical Lead in each PCN to deliver a coordinated and high-quality Social Prescribing Link Worker service that delivers a range of wellbeing and social prescription services within Hounslow PCNs – supporting residents to better self-manage and engage with the
extensive range of support in the community.

The Social Prescribing Programme Manager will support the development of Social Prescribing in Hounslow, represent and promote the service to stakeholders, develop integrated ways of working with providers and monitor and evaluate the effectiveness of the service against agreed targets.

The Link Worker programme is a national initiative from NHS England and NHS Improvement, and we are seeking an individual with a background in health, care, public health or voluntary sector services.
