Salary: £29,503 (plus contributory pension)

Hours: 37.5 hours per week

Accountable to: Team Leader

Based: A Primary Care Network in Hounslow, within a specific lead GP Practice

Contract duration: open contract (on completion of probation)

Social Prescribing Link Workers (SPLW) – four posts

Link Workers will provide connections between the GP and community services that can support residents to address their wider health and care needs.

The Link Worker programme is a national initiative from NHS England and NHS Improvement and we are seeking individuals with a background or interest in health, care, public health or voluntary sector services.
